Background Consent Form
For the safety of those attending our campus, there are some volunteer positions at our church which require individuals to have a basic background check. Examples of these positions include anyone working with children, the church database or managing financial information. If you are applying for any of these positions please fill out the background consent form online.
Driver Insurance Information Form
Our insurance company requires us to qualify any driver (paid or volunteer) who drives on behalf of McLean Bible Church. In keeping with our responsibility to provide safe and responsible drivers, we must qualify each staff and volunteer driver. If you are applying to drive the MBC PW mini bus, please fill out the Driver Info Form.

Child Dedication Application
If you would like to dedicate your child to the Lord during a worship service at the Prince William Campus, please review the Child Dedication page, fill out the application form, and make plans to attend the next child dedication class.
